The Queens EOC online application must be completed and submitted for admissions consideration to participate in any EOC program. The application can be completed in-person, with staff assistance as necessary, at the Queens EOC Admissions Office, located at 158-29 Archer Avenue, Jamaica, NY (between 160th Street and Parsons Blvd. above the MTA subways station where the E, J and Z lines end on Parsons Blvd. and Archer Avenue), Monday – Wednesday, 9:30 AM – 2:00 PM and Thursday, 2:00 – 4:30 PM. Per SUNY/CUNY COVID Vaccination policies, effective February 1, 2023, CUNY COVID-19 policies require that visitors only present valid proof of identification to enter the Queens EOC. Please bring an actual photo identification for your visit.
The online Queens EOC application can be completed and submitted by visiting the Queens EOC webpage at https://queenseoc.net and selecting the APPLY ONLINE button that is on the homepage. It will take you to the EOC application. Be sure to SELECT the Queens EOC as the EOC you want to attend, as well as your program choice from the drop-down menus. Please complete the entire application. It will start the admissions team working on your application right away (the online application is accessible 24/7).
